Trimming a doorway that’s at an angle to its adjacent walls

I’ve done half a dozen different searches across both Google and Bing and haven’t found a set of terms that yields and answer to this specific question. Every article I’ve found for variations of “doorway trim odd angle walls” or “how to trim doorway at an angle to adjacent walls” – fail to address this particular circumstance.

I’ve got a doorway set 45° and flush to its adjacent walls. As it is, the vertical trim pieces are neither flush to the wall nor at a proper 90° to the door frame they’re supposed to be attached to, so the top trim won’t meet up cleanly at all. I’d like to not use decorative blocks since it would be inconsistent with the rest of the house, so how would you handle it? Assume off-the-shelf trim from the local box store – the plain stuff with the simple curved surface. (I’ve even considered 3d printing a transition piece!)
