This is a case for the Joystick Shield v1.a with an arduino UNO.
Have a look at the code I made to send commands to my MPCNC (Marlin)

I had some problems with some taps breaking off (turned out my 3d-printer needed calibration…), and had to cut off one of the corners of the shield, but overall it works as expected. Ended up gluing it together to secure everything in place… exported a STEP-file of the cad-model so you can make modifications as you like – please let me know if you do!
No support needed when printing!
